29 CFR 1926.417(a): Controls that were to be deactivated during the course of work on energized or de-energized equipment or circuits were not tagged. On or about May 15, 2023, and at times prior to, controls within the series circuit for taxiway lights that needed to be de-energized were not tagged, exposing employees to electrical hazards for the following instances: a. Taxiway A (alpha), and b. Taxiway L (lima).
