29 CFR 1910.178(l)(4)(iii):An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years. This is applicable to shipyard employment by 29 CFR 1915.120 that incorporates the General Industry requirement: Establishment - The employer failed to complete forklift operator evaluations at least every three years. The employer could not provide records to show when the last evaluation was completed and no employees could recall completing the evaluations in the past three years during the interviews.
