29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:) a) Production Areas (Facility - Fabrication/Finishing Department): On or about May 08, 2023, and at times prior, the employer exposed employees to amputation hazards in that procedures were not developed, documented and utilized for employees performing service and maintenance activities such as but not limited to "end of day" equipment cleaning on the AccurShear (Model No.: 625010 Serial No.: 6637).
