29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment: a) Chicago Booth Manufacturing, Inc. located at 5000 W Roosevelt Rd., Chicago, IL 60644: On or about March 29, 2023, the employer failed to certify that a workplace hazard assessment had been performed. Employees were exposed to hazards that required the use of personal protective equipment such as eye protection, ear protection, hand protection, and respiratory protection.
