29 CFR 1910.119(d)(3)(ii): The employer did not document that the equipment complies with recognized and generally accepted good engineering practices. Workers in the Cumene Plant at INEOS Phenol located in Pasadena, Texas, were potentially exposed to hazards when the employer did not document that LPG/propylene unloading arms were constructed in accordance with INEOS Phenol Pipe Specification #11.
