Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification. a) On or about January 11, 2023 at 261-267 Washington Blvd., Oak park, IL 60301, the employer had taken a triple outlet extension cord, three way plug and extension cords and connected them together not in accordance with the listing and labeling of the equipment. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
