29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment. a) On or about November 20, 2022, at the previously addressed inspection site, employees adjusting wooden pallets at the pallet infeed magazine and palletizer of the Poly 12 stacker were exposed to unexpected energization, start-up, or release of stored energy because the machine or equipment was not turned off or shut down before adjusting the wooden pallets.
