29 CFR 1910.133(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: (a) On or about October 4, 2022, the employer did not ensure that each affected employee in the pre sort screen utilized face shields or eye protection with side shields to prevent flying particles such as glass shards, metals, and wood chips from entering the eyes. This exposed employees to eye injury and loss of vision. (b) On October 4, 2022, the employer did not ensure that each employee performing grinding operations utilized face shields or eye protection with shields to prevent flying particles and liquid chemicals from entering the eyes. This exposed employee to eye injury and loss of vision. (c) On October 4, 2022, the employer did not ensure that each employee performing welding operations utilized face shields or eye protection with shields to prevent flying particles, liquid chemicals, and injurious light radiation from entering the eyes. This exposed employees to eye injury and loss of vision.
