29 CFR 1910.254(d)(9)(iii): Cables with damaged insulation or exposed bare conductors were not replaced. On or about July 19, 2022 and at times prior to, employees used welding leads with damaged insulation and exposed bare conductors to perform welding operations, exposing employees to shock hazards.
