On or about May 9, 2022 at the Gordon Food Services facility in Houston, Texas, workers were exposed to chemical hazards of anhydrous ammonia when the employer did not establish and implement written procedures to maintain the on-going integrity of process equipment: a) The employer did not develop and implement procedures for maintaining the integrity of the Diffusion tank. b) The employer did not implement procedures for maintaining the integrity of the Low Temperature Return suction piping. c) The employer did not implement procedures for maintaining the integrity of Medium Temperature Return Suction piping.
