29 CFR 1910.178(l)(1)(i):The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) Rumpke Waste & Recycling Services located at 431 Prospect Dr. Findlay OH 45840: On or about April 11, 2022, the employer did not ensure employees were competent to operate a powered industrial truck through completion of training and evaluation. A untrained operator using a Hyster 60 forklift to handle the lifting arm of a McNeilus refuse truck, exposed employees working in and around the area to struck by, caught in, and crushing injuries.
