29 CFR 1910.134(g)(2)(iii): The employer did not repair or replace the respirator before allowing an employee to return to the work area when the employee detected vapor or gas breakthrough, changes in breathing resistance, or leakage of the facepiece: (a) On or about April 5, 2022 in the MR3 Unit at the Covestro chemical manufacturing facility in Baytown, Texas, worker was exposed to chemical fumes when the employer did not repair or replace the respirator before allowing its employee to return to the work area after the worker had experienced an unexpected disconnection of the respirator from a damaged airline which caused a change in breathing resistance of the respirator and caused the worker to remove the respirator while in the work area where a hazardous atmosphere was present.
