29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices. a) On or about December 14, 2021, at the ore unloading station/elevator number 3, the employer failed to provide adequate training for employees performing service and/or maintenance activities to ensure the knowledge and skills required for the safe application, usage and removal of energy control devices were acquired. An ore unloading employee sustained fractures to the right arm, contusions, and lacerations to the face while attempting to clean ore with a brush from the wheel of the conveyor while it was still operating, not de-energized or locked out.
