29 CFR 1910.333(a)(1): Live parts that the employee was exposed to were not de-energized before the work began in or near them. On January 18, 2022, the employer did not ensure the transformer was de-energized prior to the employee working near an electrical breaker box exposing the employee to arc flash hazards.
