29 CFR 1910.132(d)(1)(i): When the employer had assessed the workplace hazard(s) and determined that hazard(s) were present, the employer did not select and/or use the types of personal protective equipment that would protect the affected employee from the hazards identified in the hazard assessment: Casino and Hotel Housekeeping, Kitchen, Janitorial Closets, Travel Center Store, and Maintenance Shop: Employees used corrosive chemicals such as Spartan NABC concentrate bathroom cleaner; Auto Chlor products including MACH Turbo, D-Grease, Super 8 cleaner, Scale Kleen, MACH Enviro Dry and QA solution; Pure Bright Bleach, Rapid Set Cement All, Arctic Ice Machine Cleaner concentrate and hazardous chemicals such as Lock Lubricant & De-icer, Oatey PVC Cement, WD40, Gorilla Glue Adhesive during cleaning and maintenance operations. Employees were not wearing personal protection equipment such as safety glasses, safety goggles, face shields, or chemical resistant gloves specific for the corrosive and hazardous chemicals used. Employees were exposed to eye burns, blindness, dermal burns, and severe skin damage.
