29 CFR 1910.178(l)(3)(i)(M): Powered industrial truck (PIT) operators were not trained in any other operating instructions, warnings, or precautions listed in the operator's manual for the types of PIT the employee is required to operate: a. On or about 12/09/2021, at the warehouse aisles 43 and 44 - forklift operators were exposed to struck-by and caught-in hazards because they were not aware they were required to maintain a minimum distance of three truck lengths between forklifts.
