29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) METAL DEPARTMENT, PLASMA TABLE - Employee(s) crawled under the plasma table to adjust and square the table, and the employee(s) did not perform lock out tag out on the energy source for the plasma table, on or about October 8, 2021. (b) METAL DEPARTMENT, PLASMA TABLE - Employee(s) performed maintenance on the plasma table, and the employer had not developed and implemented machine specific energy control procedures for the plasma table, or about October 8, 2021. (c) METAL DEPARTMENT, HOLE PUNCHER - Employee(s) performed maintenance which included cleaning and greasing on the General Electric C45 Bliss power press, and the employee(s) did not perform lock out tag out on the energy source for the power press, on or about October 8, 2021. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
