29 CFR 1910.29(b)(1): The employer did not ensure that guardrail systems met the requirement that the top edge height of top rails, or equivalent guardrail system members, that are 42 inches (107 cm), plus or minus 3 inches (8 cm), above the walking-working surface: On or about October 12th, 2021, and at times prior thereto, the employer did not repair and maintain the top guardrail to ensure the height of the rail was within three (3) inches plus or minus of the required height of 42 inches on the tumble-back platform, exposing employees to fall hazards.
