29 CFR 1910.147(c)(4)(ii):The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items of this section. a. ArcelorMittal Tubular Products USA LLC - Shelby, Ohio: On or about September 30, 2021, the employer did not ensure that energy control procedures for the West Bench Push Pointer identified specific steps to follow to render the electrical, hydraulic, and pneumatic energy sources safe in that it lacked specific steps for shutting down and isolating energy sources, placement of lockout devices, and for verification of isolation. Employees were exposed to crushing and struck-by injuries when unjamming and performing setup tasks. b. ArcelorMittal Tubular Products USA LLC - Shelby, Ohio: On or about October 6, 2021, the employer did not ensure that energy control procedures for the West Bench Entry identified specific steps to follow to render the electrical and hydraulic energy sources safe in that it lacked specific steps for shutting down and isolating energy sources, placement of lockout devices, and for verification of isolation. Employees were exposed to pinch point and struck-by injuries when setting up the die and mandrel. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
