29 CFR 1910.146(d)(6): The employer did not provide at least one attendant outside the permit space into which entry is authorized for the duration of entry operations a) Omnisource LLC is located at 1500 Old Bowman Street Road Mansfield, OH, 44903: On or about September 9, 2021, an employee entered a permit-required confined space, Harris Baler Model TG 2102 to perform welding on a baler ram and was not provided at least one attendant outside the permit space. Employees were exposed to crushing hazards in the event that the unsecured internal baler ram fell.
