29 CFR 1910.179(b)(8): Employee(s) other than designated personnel were permitted to operate cranes: a. Liberty Oilfield Services, Inc., 9540 E. 104th Ave Henderson, CO 80640: On or before July 28, 2021, the employer did not ensure that workers performing maintenance on top of a Hydraulic Horse Power Pump Trucks, were protected from struck-by hazards from movement of the AmQuip 15 ton Bridge Crane in Bay 1. The crane operator was not qualified or trained to identify hazards and moved the bridge from one end of the bay to the other without looking for or alerting employees who were working in the path of the moving crane. A worker was struck by the crane's bridge and fell to the ground. Abatement Note: Feasible and acceptable abatement methods include: Crane Operator Training per ASME B30.2-16 section 2-3.1, properly trained persons designated by management authorized to operate the radio controlled crane per the per the American Equipment LLC Operation and Maintenance Manual; Product Manual Safety Information in Radio Controlled Safety section 2.3
