29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Pit Mounted Centrifugal Casting Machine "A Spinner": On or about April 26, 2021, the employer did not ensure lockout procedures were utilized to protect employees working at, on, or around the spinner from unexpected start-up and operation while performing tasks associated with the set up and maintenance of the spinning machine. This exposed employees to caught-on and crushing hazards. (a) Pit Mounted Centrifugal Casting Machine "B Spinner": On or about April 26, 2021, the employer did not ensure lockout procedures were utilized to protect employees working at, on, or around the spinner from unexpected start-up and operation while performing tasks associated with the set up and maintenance of the spinning machine.
