WORKSITE: The Nissan Model Optimun 50 Powered Industrial Truck, Model # CPJ02A25Eand Serial # 9W1803, was modified with a permanently affixed platform attachment that affected the safe operation of the Powered Industrial Truck without the manufacture's prior written approval.
