29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: On or about October 09, 2020, and at times prior thereto, at a workplace located at 8758 South U.S. Hwy 377, Dublin, Texas; employees were operating a Baldor Buffer s/n B1806120149 with no work rest installed exposed to finger amputation, cuts to hands, and severe lacerations.
