29 CFR 1910.147(c)(4)(ii)(B): Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy had not been created and implemented: a) At the facility, on or about September 3, 2020, the employer failed to develop equipment specific procedural steps for isolating and securing machines from gravity, in that the lock-out pin was not identified as a lockout point in accordance with the manufacture's user's manual, thereby exposing employees to crushing, struck-by, and caught-in hazards.
