29 CFR 1910.119(d)(3)(ii): Compliance with recognized and generally accepted good engineering practices (RAGAGEP) has not been documented for the equipment in a covered process: (a) Fruit Cooler Storage Room: On or about April 3, 2020, the employer failed to document compliance with employer's chosen recognized and generally accepted good engineering practices (RAGAGEP), which included the "International Institute of Ammonia Refrigeration's Standard ANSI/IIAR 2-2014 [RJW-O1] [SL-O2] Safe Design of Closed-Circuit Ammonia Refrigeration Systems, Section 13.4.2 which states, "Refrigerant piping shall be isolated and supported to prevent damage from vibration, stress, corrosion and impact", and Section 7.2.4 which states, "Physical Protection. Equipment shall be protected where a risk of physical damage exists. Where equipment containing ammonia is located in an area with heavy vehicular traffic during normal operations and a risk of impact exists, vehicle barriers or alternative protection shall be provided in accordance with the Fire Code", where the small diameter liquid ammonia piping connected to the evaporator (AAU-7) was not supported and protected from physical impact damage during pallet loading/unloading operations. This hazardous condition exposed workers to toxic and corrosive hazards when a powered industrial truck struck ammonia piping resulting in an anhydrous ammonia release.
