29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person. Newbrook LP on or about February 03, 2020, at the address of 1559 Falcon Cove Rd., Waterville, OH: The employer did not ensure the scaffolding platform (fabricated from OSB board) used for employees as a working surface, was designed by a qualified person. Employees were installing sheathing material to the roof of the structure and was exposed to a maximum fall of approximately 18 feet.
