29 CFR 1910.147(c)(4)(ii)(B): Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy had not been created and implemented: On or about January 20, 2020, the employer failed to ensure specific procedures were in place to control energy sources prior to working on the wheeled Caterpillar loader model 938M. Employees were trying to repair a suspected hydraulic hose line leak. Employees were exposed to an unexpected start-up and/or unexpected movement of the loader. Employees exposed to a crush-by hazard.
