29 CFR 1910.146(d)(4)(i): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not provide, at no cost to the employees, testing and monitoring equipment needed to comply with 29 CFR 1910.146(d)(5): On or about January 11, 2020 the employer did not provide testing and monitoring equipment for employees cleaning a heater treater system at the Dalziel Lease Kenneth Scott Heater prior to entry into the tank vessel system.
