29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. On or about November 12, 2019, and at times prior thereto, employees in the machine shop were required to utilize metal lathes to rotate steel shafts at speeds of up to 500 RPM in order to smooth them out and reduce their outer diameter. The shafts were occasionally positioned in the lathes in such a manner that they would protrude out of the back-end of the lathe and into unguarded areas near common walkways through the Machine Shop. No barriers or form of guarding existed to prevent employees in the area from walking into or coming in direct contact with the shafts during lathe operation, exposing employees to the hazard of getting caught in rotating parts.
