29 CFR 1910.119(f)(1)(ii)(A): The employer did not develop and implement written operating procedures that addressed the consequences of deviations from the operating limits in the covered process; Avantor Performance Materials/Philipsburg, NJ/Building 101-Methanol Distillation Process Area: a) On or about October 2, 2019, the employer's operating procedure WI-00244 - Solvent Distillation did not include consequences from loss of nitrogen during Methanol distillation, exposing employees to fire and explosion hazards in the event of a Methanol release. b) On or about October 2, 2019, the employer's operating procedure WI-0024 - Solvent Distillation failed to identify the consequences from loss of agitation during Methanol distillations, exposing employees to fire and explosion hazards in the event of a Methanol release.
