29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees: a) 1524 N. Philip St., Philadelphia, PA 19122 - The employer did not ensure a cover installed over a floor hole, at a height of approximately 12 feet 9 inches above the lower level, was secured, thus exposing employees to fall from height hazards. This condition was noted to exist on or about April 6, 2019.
