29 CFR 1926.960(c)(1)(i): The employer did not establish minimum approach distances (MAD) no less than the distances computed by Table V-2 for ac systems: At 6061 SW 80th Street, SOUTH MIAMI, FL, 33143: On or about September 21, 2019 and at times prior, a Lineman was exposed to an electrocution hazard when working from an elevated position, inside a bucket of an aerial lift, in close proximity to overhead energized electrical lines. NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
