29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about September 3, 2019: a. In building 10, north wall, where an employee was exposed to caught-by hazards while operating a Mori Seiki brand, CNC lathe, company ID S327, without interlocks to guard the rotating chuck. b. In building 10, west end center of the shop, where an employee was exposed to caught-by hazards while operating a Sharp brand, 1660L manual lathe, company ID C119, without a guard to cover the rotating chuck. c. In building 10, southwest corner of the shop, where an employee was exposed to caught-by hazards while operating an Ooya brand, RE-2M vertical mill, company ID M409, without a guard to cover the rotating chuck. d. In building 10, southeast area of the shop, where an employee was exposed to caught-by hazards while operating an Ooya brand, RE-1M vertical mill, company ID S402, without a guard to cover the rotating chuck. e. In building 11, north wall of the shop, where an employee was exposed to a caught-by hazards while operating a Rockford Machine Tool Co. brand 28 inch Hydraulic shaper, company ID T903, without a guard to cover the transversing part. f.
