29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) This violation occurred on or about July 26, 2019, and at times prior thereto; when employees operating a Komatsu forklift truck #30, model FG30HT-12, SN 564318A that was equipped with additional fork truck attachment/boom lift which affected the capacity and safe operation of powered industrial were exposed to a tip-over hazard.
