29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about July 17, 2019 and at times prior to, at the location of 1331 Kelly Ave, Akron OH 44306, on the production floor, employees required to removed jams caused by hardened, cooled polymer from the mouth of the plastic extruder of Machine # EX-2 (Serial Number 4541), were exposed to thermal hazards when heated, melted plastic had built up behind the hardened plastic.
