29 CFR 1910.303(b)(7)(iv): There were damaged parts that may adversely affect safe operation or mechanical strength of electric equipment, such as parts that were broken, bent, cut or deteriorated by corrosion, chemical action, or overheating: a) On or about 05/14/19, in the Loading dock (Door #2), employees were exposed to electrical shock when using a defective outlet to plug in the lamp and the portable conveyor belt. The openings in the outlet where worn out to the extent that it was possible to plug in the connector plug wrong, energizing the ground plug and plug in other connector plugs not designed for that outlet. NO ABATEMENT CERTIFICATION REQUIRED
