29 CFR 1910.147(c)(7)(i)(A): Each authorized employee had not received training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: a. On or about April 29, 2019, the employer failed to ensure that operators who were tasked with unjamming equipment and removing stuck parts were trained as authorized employees to perform such tasks. b. On or about September 4, 2019 a maintenance employee was working on Press #2 due to an electrical issue and had not applied appropriate locks to all energy sources, including the hydraulics. The employee was identified as an authorized employee but had not received appropriate training by the company.
