29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: a. On or about May 13, 2019, an employee performing maintenance activities from plank in the pit area of the Line 1, Drive Unit #4 in Department 412 was exposed to fall hazards of approximately 8 feet to the bottom of the pit. b. On or about May 13, 2019, employees performing maintenance activities in the pit area containing the Line 1, Drive Unit #4 in Department 412 were exposed to fall hazards of approximately 8 feet because an approximately 24 inch gap in the guardrails was used to access a wood plank that spanned the distance above the pit to the drive motor. The employer did not ensure fall protection was used at the gap(s) in the guardrails around the pit.
