29 CFR 1910.132 (a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: Railcar Cleaning Terminals: On or about May 10, 2019, and at times prior thereto, employees were exposed to a chemical splash hazard while performing cleaning and washing operations inside railcar tankers with commodity without adequate protective coverings such as, but not limited to, a full-body impervious suit or a chemical resistant suit.
