29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about April 17, 2019, employees applying established procedures for service work of a jaw crusher did not have the knowledge of the type and magnitude of the gravitation energy of a the flywheel (1910.147(d)(1)), did not have all energy devices needed to control the flywheel (1910.147.(d)(3)), and did not ensure that all potentially stored energy of a counter weight was relived, disconnected, or rendered safe (1910.147(d)(4)).
