29 CFR 1926.200(g)(2): All traffic control signs or devices used for protection of construction workers did not conform to Part VI of the Manual of Uniform Traffic Control Devices (AMUTCD"), 1988 Edition, Revision 3, September 3, 1993, FHWA-SA-94-027 or Part VI of the Manual on Uniform Traffic Control Devices, Millennium Edition, December 2000, FHWA, which are incorporated by reference. a. Vaughn Industries, Inc. worksite located on County Road 40 in Sullivan, Ohio: On or about April 9, 2019, the employer did not ensure the transition zone in the west bound lane was properly identified with traffic cones to channel traffic into a different lane. Employees performing flagging operations in the work zone were exposed to struck-by hazards from vehicular traffic due to the improper channeling of west bound traffic. b. Vaughn Industries, Inc. worksite located on County Road 40 in Sullivan, Ohio: On or about April 9, 2019, the employer did not ensure the proper placement of warning signs prior to the work zone to warn west bound traffic of the work zone and flagging activities. Employees performing flagging operations in the work zone were exposed to struck-by hazards from vehicular traffic due to the improper placement of warning signs. One feasible and acceptable method to correct this hazard is to develop and implement a site specific traffic control plan which identifies explicit procedures for traffic control and flagging operations. This plan should consider and reference factors such as but not limited to: set up of the work zone including transition zones, flagger locations, placement of warning signs, and work zone inspections according to ANSI A10.47 - 2009 Work Zone Safety for Highway Construction and the Manual for Uniform Traffic Control Devices 2000 ed.
