29 CFR 1910.218(a)(2): The employer did not maintain all forge shop equipment in a condition which will ensure continued safe operation: (a) Worksite: On or about 3/25/2019, the employer did not ensure that the Drop Hammer machine was properly maintained, which resulted in the sling arm (which holds the hammer) slipping off the shaft that drives it, allowing the hammer to fall into the point of operation, and striking the right hand of the victim.
