Text For Citation: 01 Item/Group: 002A Hazard:
29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: (a) On February 26, 2019, the acting shift superintendent was observed wearing a short sleeved t-shirt while unloading parts from a 900 +/- degree F furnace with a crane and opening and closing the furnace lid. (b) On or about February 13, 2019, an employee received third degree burns while wearing shorts under the protective aluminized coat while quenching aluminum from a 985 degree furnace in a 210 degree quench tank. (c) On or about October 26, 2018, the plant superintendent was not wearing safety glasses or a face shield and received second degree facial burns when a flame curtain inside the furnace ignited as the furnace door was opened and the fire flashed on the two employees who were checking to see if the furnace was ready to load from approximately twenty feet away. (d) On or about October 26, 2018, an employee trainee was not wearing safety glasses or a face shield, or a long sleeved 100% cotton uniform shirt and received second degree burns to the face, neck and upper chest, when a flame curtain inside the furnace ignited as the furnace door was opened and the fire flashed on the two employees who were checking to see if the furnace was ready to load from approximately ten feet away. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
