29 CFR 1926.502(d)(19): Personal fall arrest systems and components subjected to impact loading were not immediately removed from service and/or were reused before they were inspected by a competent person and determined to be suitable for reuse: On or about January 23, 2019, at 1039 Gulf Drive, Summerland Key, Florida, 33402, an employee was exposed to a 20-foot fall hazard when applying roofing materials onto asteep slope (6:12) roof, while wearing a personal fall arrest system, wherein the lanyard was previously subjected to impact loading and not immediately removed from service. NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
