This violation was most recently observed on or about January 8, 2019, in the Refrigeration Process Unit ER #2 at Americold Logistics located in San Antonio, Texas, when the employer did not ensure the alarm system worked properly to provide a supervisory alarm for the continuous (non-emergency) exhaust fan failure.
