Text For Citation: 01 Item/Group: 002A Hazard:
29 CFR 1910.132(d)(1): The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). If such hazards are present, or likely to be present, the employer shall: 29 CFR 1910.132(d)(1)(i): Select, and have each affected employee use, the types of PPE that will protect the affected employee from the hazards identified in the hazard assessment. a. On and/or before January 7, 2019, in the injection area, the employer did not select and have each affected employee use the types of PPE including face shields and heat resistant long sleeve protective clothing that would protect the affected employee performing activities such as clearing/cleaning out solidified nylon plastic material and Asaclean from the feed throat and barrel after the feed hopper was removed on the I33 Haitian injection molding machine (model: MA6000; S/N: 201107060034995). On or about January 7, 2019, an employee performing the aforementioned task received burns on the face, neck, and arm from liquefied nylon, gaseous, and/or other material when it was released through the feed throat due to pressure buildup.
