29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: a) On or about December 20 and 21, 2018, the employer did not provide fall protection on the perimeter of salt to salt quench tank on line 2 with a fall hazard of approximately 5.9 feet to the exterior of the tank. b) On or about December 27, 2018, an employee without fall protection was exposed to a fall hazard of 14 feet while tightening motor cover bolts above line 1. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
