29 CFR 1910.333(c)(3): When work was to be performed near overhead lines, the lines were not deenergized and grounded, nor were other protective measures provided before work was started: A.) Brad's Fuel Filtering, Inc., 801 Butler Pike, Mercer, PA 16137, west side of the prison facility, on or about December 13, 2018 and times prior thereto: An employee raised a metal stinger/siphon tube in the air, came into contact/proximity with 14.5-kV overhead powerlines, and was subsequently electrocuted. The powerlines were not deenergized and grounded, or otherwise provided with protective measures.
