Text For Citation: 01 Item/Group: 002A Hazard:
29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling: a) Warehouse - on or about November 7, 2018, the employer did not ensure that an employee operating a Raymond EASi order picker truck, working from the platform at heights greater than four (4) feet from grade, was protected from falling with the use of a guardrail system, safety net system, or personal fall arrest system. b) Warehouse - on or about November 7, 2018, employees operating Raymond EASi order picker trucks for order-picking were required to use a lanyard and body belt which was not an approved system and did not protect the employees from falling from the platform at heights four (4) feet and greater from grade.
